Home health industry leader HealthWyse, LLC, today announced a strategic collaboration with Sutter Care at Home (SCAH).

The agreement calls for SCAHs Integrated Chronic Care Model team to consult in the ongoing development of HealthWyses chronic care management solution.

SCAHs Beth Hennessey and Paula Suter are award-winning experts who developed the Integrated Chronic Care Model. This collaboration is designed to further the goals of improving patients quality of life and delivering satisfaction to clinicians. This will be accomplished through comprehensive technology support for best practice at point of care.

We are pleased to be working with HealthWyse on such an important initiative, said Hennessey. When were training executives across the country, we consistently hear a need expressed for supportive technology that includes robust chronic care management functionality.

In addition to collaboration on product development, SCAH will provide Integrated Chronic Care Management training and certification for users of the HealthWyse solution. This train the trainer model will serve to disseminate critical knowledge throughout the home care industry.

By ensuring that clinicians are properly trained in chronic care methodologies, and with the use of supporting software, agencies can expect to see marked and consistent outcome improvements.
